Public health is everyone’s responsibility. USC works together with local governments and community members to create a robust public health program for the USC community and its neighbors. The Office of Environmental Health & Safety (EH&S) shepherds USC’s public health program with guidance, best practices, and resources for issues that may impact the campus population and surrounding community.

USC Campus Health Communicable Disease Reporting Form

Complete the USC Campus Health Communicable Disease Reporting Form if you are a USC department representative or student that has been made aware of communicable illness in a section of the USC campus environment. Do not include names or identifiable information about specific individuals, which would be protected health information.
