Laboratories must be safely prepared prior to relocation or close-out and the responsible principal investigators (PIs) must ensure safe conditions during the process. 

Note that lab spaces and equipment are required to be decommissioned prior to leaving the University, relocating, or renovating a laboratory. Decommissioning is also required prior to relocating laboratory equipment or sending laboratory equipment for disposal, repair, maintenance, or calibration.


First Steps

  1. Initiate dialog with EH&S HazMat by first completing the Lab Closeout Request Form.
  2. Meet with EH&S representatives for an initial survey of lab(s). EH&S will schedule the meeting and prepare a closeout plan based on the meeting.
  3. Follow instructions in the Lab Move or Close-Out SOP in tandem with the Lab Move or Close-Out Checklist to address and secure all items (e.g., transfer or disposal of chemicals, biomaterials, controlled substances, and radioactive materials; decontamination of lab spaces and equipment) prior to relocation or closure.
  4. Contact hazmat@usc.edu or (323) 442-2200 at any time during the process if questions arise.
