Suspected theft or significant loss of controlled substances and precursor chemicals must be reported immediately to the entities below. This must be completed within 24 hours of the reported loss.

After notifying EH&S and DPS, complete and submit the Controlled Substance Incident Report.
EH&S will work with labs to ensure all necessary reports are submitted to the DEA (Form 106). In addition, DPS will file a police report to document the theft or loss. Theft or loss of any significant amount of precursor chemical must be reported by EH&S to the CA-DOJ within three days of the discovery of any theft or loss. Reports will be kept confidential to the extent permitted by law and University policies.
