Emergency
- Notify the Department of Public Safety (DPS) at (213) 740-4321 or (323) 442-1000. For a non-emergency, dial (213) 740-6000.
- Notify EH&S immediately at (323) 442-2200 or ehs@usc.edu to report the injury/incident.
- Notify the supervisor.
Non-Life-Threatening Workplace Injury or Illness
Faculty, Staff, or Volunteers
- Immediately report any injury or illness to your supervisor, Principal Investigator (PI), or HR Partner.
- Contact USC’s workers’ compensation administrator, CorVel, by calling (833) 202-5216. A nurse will triage and help you locate an approved medical facility for treatment.
- Submit completed Workers’ Compensation Claim Form DWC-1 to your supervisor or HR Partner.
- After each medical appointment, provide work status updates to your HR Partner and CorVel.
USC Students
- Immediately report any injury or illness to your supervisor if you are a student worker, graduate student, or working in a lab

- Speak to an appointment representative and follow instructions.
NOTE: If calling after business hours, students will be transferred to an on-call physician or nurse.
Supervisors
- Find out what happened – you are the first line of defense.
- Instruct employee to contact USC’s workers’ compensation administrator, CorVel, by calling (833) 202-5216.
- Complete a Manager’s Report of Incident Form (or Volunteer’s Injury or Illness Report) and the Workers’ Compensation DWC-1 form.
- Send the completed form to your HR representative.

If any injury or illness (Faculty, Staff, Students, Volunteers, or Visitors) occurs inside a laboratory at USC, notify EH&S at ehs@usc.edu.
Incident Reporting
Cal-OSHA regulations require that employers must immediately report any work-related death or serious injury or illness. Immediate means as soon as practically possible but not longer than 8 hours after the employer knows or with diligent inquiry would have known of the death or serious injury or illness. EH&S personnel will make notifications to the Cal-OSHA District office as required.Cal-OSHA defines a “serious injury or illness” {T8 CCR 330(h)} if the employee:
- Requires hospitalization for a period in excess of 24 hours for other than medical observation; OR
- Suffers a loss of any member of the body; OR
- Suffers any serious degree of permanent disfigurement.
