The California Department of Public Health (CDPH) requires registration of all X-ray producing devices within thirty (30) days of installation. Registration fees are every two years.
Use the form below to register all USC devices that produce radiation (e.g., X-rays or gamma rays); EH&S will assist with the registration and maintain a copy of necessary paperwork.
CDPH registration fees are the responsibility of the research group, department, or the school. Note that penalties may apply for late or delayed payment. Radiation Safety will notify the researcher about upcoming registrations.
Contact radsafety@usc.edu if an instrument is slated for decommission. Registration fee and/or late fee can then be avoided.
